This bill changes how Medicare pays for certain expensive surgical procedures when they're performed in a doctor's office instead of a hospital or surgical center. Specifically, it targets surgeries that use very costly medical supplies—things like specialized implants or devices that cost more than $500. Starting in 2025, when these procedures are done in an office setting, Medicare will pay the doctor's office using a new formula that's based on what would be paid if the same surgery were done at an ambulatory surgical center (a standalone outpatient surgery facility). This is meant to create more consistent payment across different settings.

The bill also protects Medicare patients from getting hit with unexpectedly high bills. When patients have these expensive procedures done in a doctor's office, their out-of-pocket costs (the coinsurance they have to pay) can't exceed the annual hospital deductible—which is currently around $1,600. If the normal payment rules would require patients to pay more than that, Medicare automatically increases what it pays the doctor's office to make up the difference, keeping the patient's share capped at that deductible amount. This means patients won't face surprise bills for these high-cost procedures.

The bill also sets up a system for the government to regularly review which procedures qualify under these new rules. Starting in 2026, Medicare can add new procedures to the list if they meet the criteria (expensive supplies, available in both office and surgical center settings), and can remove procedures if the supply costs drop below certain thresholds. Doctor's offices that want to participate in this program have to meet safety standards and agree to accept Medicare's payment as full payment—they can't bill patients extra amounts beyond what Medicare allows.
