# Understanding the Comprehensive Care for Alzheimer's Act

This bill creates a new Medicare program specifically designed to help people with Alzheimer's disease and other forms of dementia receive better coordinated care. Right now, people with dementia often see multiple doctors and specialists who don't always communicate with each other, leading to confusion, unnecessary hospital visits, and poor outcomes. This bill aims to fix that by paying healthcare organizations to provide comprehensive, organized care management for Medicare patients with dementia.

The program works by having healthcare organizations—like hospitals, doctor's offices, and health systems—take responsibility for managing all aspects of a dementia patient's care. These organizations would assign patients to different care levels based on how complex their situation is. Someone with early-stage dementia and few other health problems would get one level of support, while someone with multiple hospitalizations and limited family support would get more intensive care coordination. The organizations would monitor patients regularly, help manage their medications, create detailed care plans, provide caregiver support and training, and coordinate with other doctors and community resources. Importantly, patients wouldn't pay anything out of pocket for these care management services—no copays or deductibles.

The program is voluntary, meaning both patients and healthcare organizations can choose whether to participate. Medicare would pay these organizations a monthly fee per patient rather than paying for individual services, which encourages them to keep people healthy and out of the hospital. Organizations that do a good job would receive bonus payments. The bill also allows for some flexibility, like providing gift cards to reward patient participation, covering telehealth visits without geographic restrictions, and paying for community services like respite care or adult day programs that help both patients and their caregivers.

This is designed as a test program to see whether this coordinated approach actually improves people's lives, reduces hospitalizations, and helps caregivers manage the stress of caring for someone with dementia. The bill also encourages healthcare organizations to reach out to minority communities to ensure the program serves diverse populations.

Comprehensive Care for Alzheimer's Act This bill allows the Center for Medicare and Medicaid Innovation (CMMI) to test a Dementia Care Management Model that provides comprehensive care to Medicare beneficiaries with Alzheimer's disease or a related dementia. Under the model, participating health care providers receive payment under Medicare for comprehensive care management services that are provided to individuals with diagnosed dementia, excluding Medicare Advantage enrollees, hospice care recipients, and nursing home residents. Required services include medication management, care coordination, and health, financial, and environmental monitoring, as well as trainings and other support services for unpaid caregivers. Providers must furnish services through interdisciplinary teams and must ensure access to a team member or primary care provider 24-7. The CMMI must set payments and determine quality measures for the model in accordance with specified requirements. The bill also allows the CMMI to design a similar model under Medicaid.
