# Rural Health Clinic Burden Reduction Act

This bill makes it easier for rural health clinics to operate and serve patients in areas where doctors are hard to find. Right now, rural clinics face strict rules about who can work there and how they have to be set up, which can make it difficult for them to hire staff and provide care. This bill loosens those rules to help these clinics function more flexibly and keep their doors open.

The main changes focus on staffing and who can provide care. Currently, rural clinics must have physicians directly involved and available, which is a major challenge in areas where doctors don't want to practice. This bill allows clinics to rely more on physician assistants and nurse practitioners—trained professionals who can handle many medical tasks—without needing a physician on-site all the time. It also lets clinics contract with these professionals rather than only hiring them as employees, giving clinics more options for finding qualified staff. Additionally, the bill removes outdated requirements that clinics must have their own laboratory on-site; instead, they just need reliable access to lab services, which could mean partnering with hospitals or other facilities.

The bill also fixes some technical definitions that were limiting where rural clinics could operate. It changes how "rural" is defined to be more accurate and creates an exception for mental health services in areas that have a shortage of mental health professionals. This means clinics in areas struggling with mental health care shortages can now offer behavioral health services even if they're in slightly more populated areas. These changes take effect on January 1, 2024, and are designed to reduce the paperwork burden and operational headaches that prevent rural clinics from hiring staff and expanding services to underserved communities.

Rural Health Clinic Burden Reduction Act This bill makes a series of changes to requirements that apply to rural health clinics under Medicare. For example, the bill allows rural health clinics that are not directed by physicians to enter into arrangements with physician assistants or nurse practitioners that generally comply with state laws (regarding scope of practice), rather than other specific requirements. It also specifies that rural health clinics (1) need only have prompt access to clinical laboratory services (as opposed to having to directly provide these services); (2) must be located in a non-urban area with a population that is less than 50,000; (3) may contract with (as opposed to employ) physician assistants or nurse practitioners; and (4) may primarily provide mental health services if located in a designated mental health professional shortage area.
