# Understanding the Strengthening Medicaid for Serious Mental Illness Act

This bill creates a new option for states to provide intensive mental health services to adults with serious mental illness while offering them extra federal funding to do so. Starting in 2025, states can choose to cover a comprehensive package of community-based mental health services through Medicaid—services like crisis teams that come to your home, job coaching, peer support groups, and help finding stable housing. The key innovation is that states no longer have to prove someone would otherwise need hospitalization to qualify for these services. Instead, if you're an adult with serious mental illness, earn less than 150% of the poverty line, and are eligible for Medicaid, you could access these intensive community supports.

The federal government sweetens the deal by increasing how much money it reimburses states for these services. Depending on how many different types of services a state offers, the federal government will cover an extra 3 to 25 percentage points of the cost. This means states have a real financial incentive to build out these programs. For example, if a state normally gets 60% federal funding for Medicaid, it could jump to 85% or even higher for these mental health services. The bill also provides $20 million in planning grants to help states develop and set up these programs.

The services themselves are designed around what research shows actually works. Assertive community treatment means a team of specialists provides intensive support 24/7, with enough staff so each person gets real attention. Supported employment helps people with serious mental illness find and keep real jobs in the community. Peer support connects people with others who've lived through similar struggles. Mobile crisis teams can respond when someone is in crisis, potentially preventing unnecessary emergency room visits or hospitalizations. The bill also emphasizes housing support, recognizing that stable housing is fundamental to recovery.

States have to meet quality standards and report data on how well these programs work, including whether they're reaching different racial and ethnic communities fairly. The goal is clear: keep people with serious mental illness living in their communities with the support they need, rather than cycling through hospitals or institutions. For people struggling with mental illness and their families, this could mean finally having access to the kind of coordinated, intensive support that helps people recover and build meaningful lives.

Strengthening Medicaid for Serious Mental Illness ActThis bill allows state Medicaid programs to cover intensive community-based services for adults with serious mental illnesses.Specifically, states may provide for coverage of services that include (1) treatment services that are available 24/7, involve multidisciplinary care teams, and that are designed to improve outcomes for adults with mental illnesses who are at high risk of homelessness, psychiatric crisis, or hospitalization or who are involved in the criminal justice system (i.e., assertive community treatment); (2) employment support; (3) peer support; (4) community-based mobile crisis intervention services; (5) intensive case management; and (6) housing support.The bill provides funds for state planning grants with respect to such coverage. It also provides for an enhanced federal matching rate, also known as the Federal Medical Assistance Percentage (FMAP), for such services.
