This bill simplifies those rules by eliminating different requirements for large versus small organizations, so everyone follows the same, simpler standards. It also removes a major barrier that forces new organizations to take on financial risk immediately—instead, organizations can now participate for three years in a lower-risk arrangement before deciding whether to take on the responsibility of repaying losses if they don't save money.

The bill also makes the system more transparent and fair by requiring Medicare to publicly explain how it calculates spending benchmarks (the targets used to measure savings) so that healthcare organizations can understand and challenge these calculations if they seem wrong. Currently, these calculations are often unclear, which can unfairly penalize organizations for achieving savings. The bill creates a formal appeal process where organizations can challenge benchmark calculations before an administrative judge within 90 days. Additionally, it establishes a new option where healthcare organizations can choose to take on full financial risk in exchange for keeping 100 percent of any savings they generate, rather than splitting savings with Medicare.

The bill also adjusts incentive payments for doctors and healthcare providers who participate in alternative payment models, gradually lowering the threshold they need to meet to qualify for bonus payments. It requires the government to provide extra support and technical assistance to rural areas, small practices, and safety net providers (hospitals that serve low-income patients) to help them participate in these programs. Finally, the bill directs the Government Accountability Office to study how alternative payment models and Medicare Advantage plans treat different types of patients and providers to identify ways to make these programs more equitable and aligned.
