# Understanding the BABIES Act: What It Does and Who It Affects

The BABIES Act is designed to address a real problem in American healthcare: many pregnant women, especially in rural and underserved areas, struggle to find maternity care because hospitals are closing their obstetric units and there simply aren't enough birthing options. This bill takes two main approaches to fix that problem by expanding access to freestanding birth centers—facilities that specialize in low-risk pregnancies and operate independently from hospitals.

The first part of the bill provides direct funding to help birth centers get started or expand. The federal government will give grants of $300,000 to $500,000 each to up to 15 birth centers per year from 2026 through 2030. These grants can be used to renovate or build facilities, buy equipment, or cover the costs of getting accredited and licensed. The bill prioritizes birth centers in areas where maternity care is hard to find or where pregnancy outcomes are worse than average, meaning rural communities and low-income neighborhoods would likely benefit most.

The second part creates a test program through Medicaid to figure out better ways to pay for birth center services. Right now, payment systems for birth centers are inconsistent and often don't cover all the services they provide. The bill gives up to six states money to plan a demonstration program, and then allows those states to run four-year pilot programs where they test new payment methods. These payment systems would need to cover the full pregnancy journey—prenatal visits, labor and delivery, postpartum care, and newborn care—rather than just individual visits. The bill also specifies that payments should include supplies like nitrous oxide for pain relief and cover situations where a pregnant woman starts labor at the birth center but needs to transfer to a hospital before delivery.

For pregnant people, especially those on Medicaid in participating states, this could mean more birthing options closer to home and potentially more personalized care. Birth centers typically have lower intervention rates and focus on natural birth when possible, which appeals to many people. However, this bill only applies to low-risk pregnancies—women with complications would still need hospital care. The bill also requires birth centers to have strong connections with nearby hospitals and physicians, so if something goes wrong, transfer to emergency care is quick and coordinated.
