This bill creates a new federal program to bring healthcare directly to homeless people living on the streets in cities and counties with significant homeless populations. Instead of requiring homeless individuals to come to clinics or hospitals, doctors, nurses, and other health professionals would go out to where people are living—on streets, in parks, or other outdoor locations—to provide medical care, preventive services, and treatment for conditions like substance use disorders.

The program works by paying healthcare providers who offer this "street medicine" through a reimbursement system similar to Medicaid. Providers can include doctors, physician assistants, nurses, therapists, and emergency medical technicians. To participate, they need to be affiliated with a hospital, nonprofit, government agency, or private practice, and they must use electronic health records to document their work. Importantly, homeless individuals wouldn't need to provide a permanent address to receive care, and they can choose any qualified street medicine provider without needing prior approval from insurance companies.

The bill also includes provisions to help recruit and retain healthcare workers for this work. It expands loan forgiveness programs for doctors and other health professionals who commit to practicing street medicine, and it creates new opportunities for community service officers (non-armed law enforcement who focus on outreach rather than enforcement) to qualify for student loan forgiveness. Additionally, the bill requires emergency shelters to provide menstrual products and security cameras to protect residents' safety and dignity.

The program is set up as a pilot that will run through 2028, with a government report due in 2027 to evaluate whether it's actually helping people move into housing, accessing treatment, and improving their overall health. The goal is to meet people where they are and provide comprehensive healthcare to one of the most vulnerable populations.
