This bill creates a test program where Medicare will pay hospitals to deliver specially designed meals to certain patients' homes after they leave the hospital. The goal is to help people recover better and avoid having to come back to the hospital.

Here's who gets the meals: Medicare patients who have diseases affected by diet (like diabetes, heart failure, or kidney disease) and who are struggling with daily activities like bathing or cooking. After they're discharged from a participating hospital, they'll receive at least two meals per day for at least 12 weeks that are designed by a nutritionist to meet their specific medical and cultural needs. They'll also get nutrition counseling. Importantly, patients won't have to pay anything out of pocket for these meals or counseling.

The program will run for 6 years and include at least 40 hospitals across the country. Medicare will track whether patients who get these meals end up healthier, have fewer hospital readmissions, and cost less overall compared to similar patients who don't get the meals. The government will pay for this program by slightly reducing payments to hospitals in other ways, so the total Medicare spending stays the same. After the program ends, Congress will get a detailed report on whether this approach actually works and should become permanent.
