This bill pushes that deadline to the end of 2026, giving people more time to continue getting care from home through video calls and phone appointments. It also expands who can provide telehealth services and where patients can be located when they use it. For mental health services specifically, the bill delays a requirement that patients have an in-person visit before getting ongoing care through telehealth, pushing that deadline from 2025 to 2027. This matters because it means people struggling with depression, anxiety, or other mental health issues can continue getting help remotely without having to travel to see a therapist or counselor first.

The bill also addresses a practical problem: making sure telehealth works well for people who don't speak English fluently. Within a year, the government will issue guidance to healthcare providers and technology companies about best practices for using interpreters during video appointments, providing instructions in multiple languages, and making patient portals accessible to non-English speakers. This ensures language barriers don't prevent people from getting care.

On the hospital side, the bill extends a program called "Acute Hospital Care at Home," which lets certain patients receive hospital-level care in their own homes instead of staying in a hospital building. This program was set to end in 2024, but the bill extends it through 2029. The bill also extends payment support for low-volume hospitals—smaller hospitals that serve fewer patients—to help them stay financially stable and keep their doors open, which is especially important in rural communities where hospitals are often the only medical facility for miles around.

The bill includes some fraud-prevention measures too. Starting in 2026, Medicare will more closely monitor durable medical equipment (like wheelchairs, oxygen tanks, and other devices) to catch suspicious billing patterns, particularly when doctors who don't have an established relationship with patients are ordering large quantities of equipment. The government will also study wearable medical devices and artificial intelligence to understand how they might help doctors make better decisions, and will investigate fraud in laboratory testing to protect Medicare from scams.
