This bill creates a new federal grant program to help small business workers get affordable health insurance through community-based partnerships. The Department of Health and Human Services will award 3 to 5 grants (each lasting 4 years) to nonprofit organizations that partner with local hospitals and small employers to provide health coverage and support services to workers who earn too much for Medicaid but struggle to afford traditional insurance.

The program targets people who work for small businesses that don't offer affordable health coverage—specifically those earning between the Medicaid limit and 400% of the federal poverty line. Instead of just providing insurance, these programs combine health coverage with wraparound services like job training, financial counseling, and health coaching to help workers improve their health and economic stability. The programs must be physically located in the communities they serve and work closely with local hospitals, employers, nonprofits, and government agencies to coordinate services and avoid duplication.

The funding model is designed to be sustainable long-term by requiring contributions from multiple sources: federal grants, state and local government, hospitals' community benefit funds, employers, and enrollees themselves. This shared responsibility approach aims to prevent dependence on any single funding source. Programs must track their success by measuring improvements in enrollees' employment, health outcomes, income, and overall economic self-sufficiency.
