# The GAIN Act of 2024: What It Actually Does

The GAIN Act is a proposal to test whether giving poor people a guaranteed monthly income improves their health. Here's how it works: the federal government would fund at least ten different local or regional programs across the country where Medicaid recipients (people who qualify for government health insurance because of low income) would receive monthly cash payments of at least $500, with no strings attached. These payments would continue for five years, and researchers would track what happens to participants' physical and mental health during that time.

The bill is based on the idea that financial stress itself makes people sick. Medical research shows that when people struggle with unpredictable income—not knowing if they'll have enough money month to month—they experience higher rates of heart disease, depression, anxiety, and cognitive problems. The bill points to a pilot program in Stockton, California that gave residents guaranteed income and found they reported better overall health as a result. This new demonstration project would test whether that same benefit works for Medicaid recipients on a larger scale.

The money would go to local governments, Native American tribes, or nonprofit organizations that apply for grants. Participants could spend the cash however they wanted—on rent, food, medical bills, or anything else—with no restrictions. The payments wouldn't count as taxable income, so people wouldn't owe taxes on the money. Each participating organization would survey participants before the program starts and then at least three times a year to measure changes in their health, employment, finances, and how they're spending the money. After five years, the government would report back to Congress on what it learned about whether guaranteed income actually improves health outcomes for low-income people.
