This bill makes changes to how Medicare pays long-term care hospitals (LTCHs)—specialized hospitals that treat patients who need extended care, often after a serious illness or surgery. The main goal is to ensure these hospitals have enough money to care for their sickest and most complicated patients without going out of business.

Right now, Medicare uses a fixed payment system for long-term care hospitals, meaning they get a set amount per patient regardless of how sick that patient is. The problem is that when hospitals treat very complex patients with multiple serious conditions, they often lose money because the fixed payment doesn't cover the actual costs. This bill addresses that by capping how much the fixed payment can increase in the near term (limiting it to $50,000 for 2025-2026, then allowing only 10% annual increases after that), while also giving hospitals extra money when they treat the most complicated patients. Specifically, hospitals will get a 5% payment boost for patients with multiple major complications, and a 10% boost for patients with even more complications. This extra money isn't supposed to come out of other healthcare budgets—it's genuinely new funding.

The bill also creates exceptions to existing rules that sometimes prevent long-term care hospitals from getting paid for treating certain high-risk patients. Starting in October 2023, hospitals can now get paid normally for treating patients with severe wounds, serious respiratory failure, severe infections, or COVID-19, even if those patients don't meet certain other technical requirements. This ensures that hospitals won't turn away or struggle to treat these vulnerable patients because of payment restrictions. The changes take effect gradually, starting in October 2025 for most provisions, giving hospitals and Medicare time to adjust.
