The Safe Step Act requires health insurance plans to create a fair and quick process for patients and doctors to request exceptions to "step therapy" rules. Step therapy is when an insurance company requires you to try a cheaper or preferred drug first before approving coverage for the medication your doctor actually prescribed.

Under this bill, patients or their doctors can request an exception in several situations: if other drugs haven't worked, if delaying treatment could cause serious harm, if the required drug causes bad side effects, if it prevents you from doing your job or daily activities, or if you're already stable on a different drug that was previously approved by another insurance plan. Insurance companies must have a clear form and process for these requests, available both on paper and online.

The timeline is strict: insurance companies must respond within 72 hours with either approval or a request for more information. If the situation is urgent and could seriously harm your health, they must respond within 24 hours. Once an exception is approved, the insurance company must cover your prescribed drug for at least one year. Insurance companies also have to report data to the government about how many exceptions they approve or deny, helping track whether the system is working fairly.
