This bill makes several changes to how health insurance works under the Affordable Care Act, with most changes taking effect between 2026 and 2032. The main goal is to lower premiums for people buying insurance on the individual market.

First, it caps how much people have to pay out of pocket for premiums based on their income. Starting in 2026, people earning less than twice the poverty line would pay no more than $10 per month toward premiums, those earning between 200-300% of poverty line pay up to $20, those at 300-400% pay up to $30, and those above 400% pay up to $40. The bill also temporarily increases the income threshold for getting premium help from 400% to 700% of the poverty line through 2031, then phases it back down. Additionally, it creates new "Healthcare Affordability Accounts" (similar to health savings accounts) where the government will deposit premium tax credits directly, and people can use these accounts to pay for health insurance.

The bill also makes significant restrictions on what health insurance plans can cover. Starting in 2026, qualified health plans cannot cover gender transition procedures, and plans that do cover abortion services must separate out the cost of that coverage so it doesn't count toward premium tax credits. States can apply for waivers to set up high-risk pools or reinsurance programs to help stabilize insurance premiums, and the federal government provides funding for these programs from 2027-2030. Finally, the bill requires photo identification for enrollment verification and makes various technical changes to how insurance exchanges operate.
