The Honor Our Promise to Veterans Act of 2025 is a comprehensive bill designed to improve how the Department of Veterans Affairs delivers healthcare to veterans. At its core, the bill sets strict timelines for getting veterans appointments: non-urgent appointments must be scheduled within seven days of when a veteran requests care or a doctor determines they need it, and urgent care appointments must be completed within 48 hours. If a VA facility can't meet these standards, it has to explain why and submit a plan to fix the problem.

The bill also expands veterans' options for getting care. It requires the VA to tell veterans about all their choices—whether that's in-person at a VA facility, telehealth through the VA, or going to a community provider—along with wait times and driving distances for each option. Veterans can choose telehealth from outside providers if the VA's wait times are too long or if they've already started treatment with that provider. The VA must update its public website weekly with current wait time information so veterans can make informed decisions.

To protect veterans from poor-quality care, the bill creates new safeguards for community providers who treat veterans. It requires background checks to keep excluded or problematic providers out of the system, establishes training requirements so community doctors understand military culture and veteran-specific health issues like PTSD and traumatic brain injury, and creates an "MST Aware" certification for providers trained in treating military sexual trauma. The bill also gives the VA's Inspector General stronger authority to audit and investigate any provider delivering veteran care.

Finally, the bill makes changes to expand the roles of certain healthcare professionals, like physician assistants, and requires the VA to learn from past contract problems to improve how it manages community care going forward. Most of these changes must be implemented within one year of the law taking effect.
