This bill tackles a major frustration for Medicare Advantage patients and their doctors: the prior authorization process. Prior authorization is when insurance companies require doctors to get approval before providing certain treatments or medications. Right now, these approval processes vary wildly between different Medicare Advantage plans, creating confusion and delays.

The bill requires Medicare to identify the most expensive treatments and medications that are ordered frequently and have clear medical guidelines, but require too many complicated steps to get approved. By January 2027, Medicare will audit these high-cost items and drugs to find the ones that are unnecessarily burdensome to approve. Then by May 2028, Medicare will create standardized approval requirements that all Medicare Advantage plans must follow for these identified treatments and drugs. This means instead of each plan having its own approval process, there will be one consistent process across all plans.

There's an important exception: doctors and providers who participate in risk-sharing payment models (like accountable care organizations) won't need prior authorization at all for these standardized items and drugs, since they're already financially accountable for treatment decisions. However, individual Medicare Advantage plans can request to opt out of this exemption if they want to keep their own approval requirements.
