# Health Center Community Transformation Hub Act

This bill gives the federal government the ability to provide extra money to community health centers so they can address the root causes of poor health in their neighborhoods. Instead of just treating people's medical problems, these health centers would use the additional funding to tackle the bigger issues that make people sick in the first place—things like not having enough food, lacking reliable transportation to get to appointments, living in unsafe neighborhoods, or not having access to good schools.

The bill defines what these "community transformation hubs" would actually do. They'd be networks of local organizations working together to help with transportation, food security, job opportunities, education, neighborhood safety, and environmental health. For example, a health center in a low-income area could partner with a food bank, a job training program, and a transit authority to help their patients get the support they need. The health centers would work with community organizations through partnerships, contracts, or agreements to deliver these services without having to do everything themselves.

To get this extra money, health centers would need to show they've studied what problems their patients actually face and explain how their proposed hub would help solve those problems. The bill also requires the federal government to provide guidance within a year on how these hubs can keep operating after the initial funding runs out, so communities don't lose these services once the grant money ends. Essentially, this bill recognizes that a doctor's appointment alone won't help someone get healthier if they can't afford food or get to the clinic, so it funds a more complete approach to community health.

Health Center Community Transformation Hub Act This bill permits the Health Resources and Services Administration (HRSA) to award supplemental funds to certain health centers that care for medically underserved populations for addressing primary social determinants of health (i.e., nonmedical factors that influence health outcomes, such as transportation, food security, and economic security). To receive the supplemental funding, health centers must be recipients of other HRSA grants that support health center operations and activities. Health centers must use the supplemental funds to implement a community-based network of organizations capable of addressing the social determinants of health. Additionally, HRSA and the Centers for Medicare &amp; Medicaid Services must issue guidance on sustaining these networks after the supplemental funding terminates.
