# The SPARC Act: Paying Doctors to Work in Rural Areas

The SPARC Act creates a loan repayment program designed to get specialty doctors and other specialized healthcare providers to move to rural communities that don't have enough of them. Here's how it works: if a specialty doctor—like a cardiologist, orthopedic surgeon, or psychiatrist—agrees to work full-time in a rural area that's short on specialists for six years, the federal government will pay off a significant portion of their student loans. Each year of service gets one-sixth of their eligible loans paid off, with the remaining balance cleared after they complete all six years. The total loan repayment can't exceed $250,000 per person.

The program also allows non-physician specialists like nurse practitioners, physician assistants, and other licensed healthcare providers to participate, though they can only receive up to 15 percent of the program's funding. Participants can have up to one year of gaps between their years of service, but they need to complete the full commitment in good faith. The loans eligible for repayment include medical school loans, federal student loans, and other education-related debt.

For rural communities, this could mean finally getting access to specialists they've been lacking—whether that's cancer treatment, heart care, or mental health services. For doctors burdened with six-figure student debt, this offers a meaningful incentive to practice in areas they might otherwise avoid. However, doctors can't use this program alongside other federal loan forgiveness programs for the same service, so they'll need to choose which option works best for them. The bill requires the government to report back to Congress every two years on whether the program is actually working and whether rural areas are seeing improvements in specialty care availability.
