It now includes people who have both Medicaid and other insurance (like Medicare or private plans), as long as the hospital's costs for treating them exceeded what all their insurance sources paid combined. This matters because safety-net hospitals often treat patients with multiple insurance sources, and the extra costs can add up quickly.

The bill also gives states a chance to go back and fix payments they made in recent years if they didn't use all their available Medicaid money. States can now increase those old payments to hospitals if they follow the new rules, and they won't have to take that money back later. This is a one-time opportunity for states to redirect unused funds to help these hospitals catch up on costs they've already incurred. The changes apply to future years going forward, but states get this special window to adjust payments from 2021 onward if they haven't already finalized their audits for those years.
